The On Form Completion? drop down box lets you select a user to be notified when the form has been completed.

The Read Receipt not understood? drop down box lets you select a user to be notified if an employee does not understand the form.

If you are unable to follow these instructions, you may not have the required user permissions. Please contact your Site Administrator.
