From the xCenta Dashboard, click on the Equipment icon in the Modules section.
A new page will appear. Click on the relevant equipment category, click the relevant piece of equipment, click the Forms tab and then click Add Form.
A new pop up will appear. Select the relevant form from the Form Template drop down, enter a DeadLineDate, allocate users and click Add Form.
