How to create a policy declaration


From the xCenta Dashboard, click on Tools > Admin in the top right of the screen. 



From the Administration screen, click on Manage Site Form Templates.



A new page will appear, click on the xCenta Templates tab.


Search for Read Declaration Template and click the  icon to copy the form template.



A new pop up will appear, change the template name to the relevant title and click Ok.



A new window will appear, click on the Appendices tab.



A new page will appear, click on the Add Appendix button.



A new pop up will appear, enter the relevant information and click Ok, for example Health and Safety Policy in the Name field and please read the attached in the Description.



Click on Add Item.



A new pop up will appear, within the Type drop down select File, upload the relevant file, click Upload File, then click Ok.



Click on the Settings tab, within the description field remove the word 'Template' and click the Save Settings button.





How to add and assign the policy


From the xCenta Dashboard, click on Tools > Admin in the top right of the screen. 


 


From the Administration screen, click on Manage Site Indexes and Forms.


 


A new page will appear. Click on New Index at the bottom of the page, select New Blank Index in the drop down and type Read Declarations in the Title, then click Add Index.



Click on the new index (Read Declarations). Click on New Item and type in the name of the document, for example Health and Safety Policy, then click Add Item.



A new sub-index will appear. Click on the index and click Add Forms, select SSOP Form in the drop down and tick the form you wish to add in, then click Next. Leave the option as This site and click Add Forms.



Re-click on the sub-index. Tick the document you wish to assign users to read, click on Bulk Assign Users at the bottom of the page, tick the names of the people you wish to read the form and tick Read Receipt. Click Assign Users once all users have been ticked.




Each user assigned the read receipt will receive an email notifying them of the form they need to read.
