There are two methods when adding a note to a form.
Method 1
Within a form in the top left hand corner, click on the Notes tab, click on the Add Note button.
A pop up will appear, enter the relevant information for your note and click Ok.
You will be taken back to the Form tab, click on the Notes tab and click Add Attachment.
A new pop up will appear, click Choose File.
A new pop up will appear, choose the relevant file and double click it then click Upload.
Method 2
Within a form, click on the Notes icon for your question.
A new pop up will appear, enter the relevant information and click Add note, then click Close once you have added your note.
You will be taken back to the form page, click back onto the Notes tab and for the note you have just created you will now be able to add an attachment.
A new pop up will appear, click Choose File, find and open the relevant document needed to be attached and click Upload.
